Student Consent Form
The student application deadline was Friday, September 18th, 2009. We are no longer accepting student applications.
The College of Design Mentor Program is a wonderful opportunity for students to develop their professional skills outside of the classroom through the guidance of a professional in the community. Students participating in this program are not only introducing themselves to the professional world, but also representing the College of Design to its alumni and friends. As a result, we have high expectations for the students who choose to participate in this program. It is important that you carefully read the below expectations to determine if you will be able to make the commitments needed to make the mentoring relationship a success.
1. Review the information on the program website or attend an information session before completing this application.
2. Participate in orientation with your mentor.
3. Initiate regular contact with your mentor via phone, email or by scheduling meetings.
4. Meet your mentor at least one time each month or more often as determined by you and your mentor.
5. Act in a professional manner at all times.
6. Return phone calls and email correspondence promptly.
7. Complete program evaluation(s) online when requested.
8. Contact the program administrator and your mentor immediately if circumstances require that you stop participation in the mentor program.
9. Notify the program office if you feel your mentor is not fulfilling his or her responsibilities.
